The following could be happening:
1. The URL is typed incorrectly in the HTML
2. The image pointed to by the URL does not exist/has been deleted
3. Incomplete path has been used - use the full URL
4. The image host has put the wrong permissions on the image.
5. The image has been generated incorrectly by a server script.
6. The URL contains special characters and has not been surrounded by quotes.
7. The URL contains spaces and % 20 has not been used as a substitution.
Can you post the code snippet with the image link, please?
